Puddles at the accessible ramp to the crosswalk remain days after there has been rain This has pushed pedestrians to cross in ways that hides them from drivers view as they are more behind parked cars as they try to avoid the puddle Not to mention this makes it not accessible for wheelchair users, people pushing strollers, etc

The soil above the sewer drain has caved in I stepped on that part of the grass and almost put my foot into the drain The drain is also not properly supported and the road is caving in The other end of the street is already has one caved in currently I put a stake in the lawn because this is a bus stop for school children and I didn't want anyone else stepping there and breaking an ankle Needs to be assessed asap

The sidewalk next to the DHS fields has layers and layers of uneven patches making an uneven surface for folks with limited mobility and snow removal

Categorizing this as Sidewalk Depression. However, it's more like Sidewalk Obstruction. The Hedges (bushes) in front of 17 West Street are now blocking 1/2 the sidewalk. Unfortunately, I have witnessed some handicapped folks on larger scooters be forced out onto the road so that they can get by this section of side walk. This has been noticed for a few years now, but seems to be becoming a safety concerns as the bushed are now much larger than they used to be.

Raised edge on sidewalk at the corner of West and Carleton. Caused a fall earlier today. The right edge is covered with dirt/debris and is very hazardous with no clear indicator of the exposed edge. Needs to be marked/painted at the very least in the meantime. Thank you.
